An In-Depth Report on Your Oral Health
Every dental exam at Palermo Village Dental is an opportunity for us to learn about your health, starting with your smile. We’ll do a Healthy Mouth Baseline Assessment with you in your exam to explore the health of your teeth, gums, tongue, jaw joints, lips, and cheeks.
This assessment gives us a rich starting point for discussing your health and any symptoms or changes since your last visit. You’re welcome to fill in the online version of this assessment via this form, or we can go through it together at your appointment.
As part of our commitment to keeping you involved in your own healthcare, we’ll provide you with a scorecard of your mouth health, explain what each measurement means, and discuss our recommended treatment plan.
